If you are making a memorial donation, please include the following in the "Notes" section:

1. The name of the person the memorial is given for 

2. The name(s) and address, as you would like them listed in the memorial letter to the family.

Example:
A memorial for Mr. John Doe given by Bill and Sally Smith, 123 Bridge Street, St. Matthews, SC 29135

This allows us to send a complete memorial letter to the family acknowledging your gift in memory of their loved one, and to mail you a confirmation letter/giving statement as well. Thank you.
